Exemptions and Waivers

Medical and Religious Exemption Requests

Students attending UMass Boston may request an exemption from any immunization requirement, subject to religious and disability accommodations pursuant to state and federal law. Note than an exemption cannot be submitted for the Tuberculosis (TB) Screening Questionnaire, as all students must complete this web-based questionnaire.

Medical Exemptions
  1. Download: Download the Medical Immunization Exemption Form. 
  2. Complete: Have your healthcare provider complete and sign the form.
  3. Upload: For instructions on uploading records, click here. If you have immunization records, please also submit these for review.
  4. Review Period: Allow time for a nurse to review your submission. If approved, your immunization status will be updated to reflect your medical exemption.

Religious Exemptions
  • To submit a religious exemption, please call 617-287-5661 to schedule an appointment with a UHS Nurse.

Meningococcal Vaccination Waiver
  • Students attending UMass Boston may waive the Meningococcal vaccine requirement. To submit a waiver, please call 617-287-5661 to schedule an appointment with a UHS Nurse.
