Daily Self Checklist
Please review this COVID-19 self-checklist each day before reporting to work. If you reply YES to one or several of the questions below, STAY HOME and follow the steps below:
- Notify your supervisor.
- Consider getting a COVID-19 test. They are free and accessible to everyone on campus. On-campus COVID-19 testing is available by appointment only. Call 617-287-5660 to book an appointment.
- Or contact your health provider.
Daily Self Checklist
1. Are you experiencing any of the following symptoms?
- Fever or chills (temperature over 100.3F) without having taken any fever reducing medications?
- Cough
- Shortness of breath or difficulty breathing
- Fatigue
- Muscle or body aches
- Headache
- New loss of taste or smell
- Sore throat
- Congestion or runny nose
- Nausea or vomiting
- Diarrhea
2. Have you or anyone you have been in close contact with been diagnosed with COVID-19, or been placed on quarantine for possible contact with COVID-19?
3. Have you been asked to self-isolate or quarantine by a medical professional or a local public health official?
If you answered YES to any of the above questions, STAY HOME, contact your supervisor, and consider getting a COVID-19 test.
We ask that employees use their best judgement when reviewing the questions. We understand that some individuals have chronic conditions, such as migraines or allergy-related coughs, that have symptoms captured on the checklist. If you have such a condition and feel that a YES response on the checklist is related to your chronic condition rather than COVID-19, you may come in but are advised to be surveillance tested regularly.
