Non Employee / Affiliate Paperwork

The Department of Human Resources has established a procedure for individuals who occasionally assist the University of Massachusetts Boston with grant functions, activities development and other services as requested. In most cases, these services require an Employee ID number and Employee ID card that would permit access and various services provided by the University (i.e., e-mail, telephone, business cards, fitness center, etc).

How do you determine the correct Non-Employee type?
CWR/Contingent Worker: A person providing services to the organization and does not have an official relationship with the organization (i.e. Consultants, Adjunct faculty, etc).
POI/Persons of Interest: A person who is not an employee or contingent worker, but is of interest to the organization such as:
- Non-HR administered employees (e.g. Volunteers, Honary Alumni, Visitors, etc)
- External Instructor/trainees in relation to the Enterprise Learning module.
- Campus Solutions persons for Student refunds

In order to gain access to these university services you must have the appointee complete a non-employee instruction packet, which includes:

Section One:

Download all forms for section one and two

1. POI (Person of Interest)/Contingent Worker (CWR) Release of Liability Form and Checklist

2. Personal Data Questionnaire:  The appointee must complete, sign and date the bottom of the form.

Section Two:

1. Guide to the Conflict of Interest Law
The appointee will check off the appropriate box to acknowledge receipt of the guide and the responsibility of understanding its contents.

2. Guide to Political Activity (Public Employees and Fundraising)
The appointee will check off the appropriate box to acknowledge receipt of the guide and the responsibility of understanding its contents.

3. Sexual Harassment Policy
The appointee will check off the appropriate box to acknowledge receipt of the policy and the responsibility of understanding its contents.

4. Drug-Free Workplace Policy
The appointee will check off the appropriate box to acknowledge receipt of the policy and the responsibility of understanding its contents.

5. University of Massachusetts Policy on Fraudulent Financial Activities
The appointee will check off the appropriate box to acknowledge receipt of the policy and the responsibility of understanding its contents.

6. University of Massachusetts Principles of Employee Conduct
The appointee will check off the appropriate box to acknowledge receipt of the policy and the responsibility of understanding its contents.
